- Admission Application/Fees
- Valid Identification [driver’s license, state ID & SSN]
- Current CPR Certification[AHA, Red Cross]
- Criminal Background check
- Tuberculosis skin test / Quantiferon TB gold / chest x-ray
- Physical and Immunization records [PNonly]
- Proof of Highschool / GED / Transcript (if applicable)
- ATI-TEA Testing Dates ( Monday through Friday 9am to 3pm starting February 2022)
